South Western Football Group Waiver/Release

This waiver/release is for South Western Football Group. Training will be $10 per session per athlete. You do not need to pay for the waiver/release, but are responsible to have payment at the time of each session. Payment will be taken at the beginning of each session. If payment is not received the athlete will not be able to train. Payment can be cash, check or card. We do not accept venmo or any other form of electronic payment.
